Student Medical Forms
Dear Parents,
As the new school year approaches, we are looking forward to a productive and healthy year for our children. Please fill out the enclosed forms and return them to the health office by the first day of school.
Please note:

A new emergency information sheet must be filled out for all students every school year.

A physical exam is required for all Kindergarten and all new students.

Scoliosis screening is conducted every 2 years starting with fifth grade. Please fill out the enclosed scoliosis permission slip for all students entering grades 5, 7, 9 and 11.

Height, weight and blood pressure will be taken for all students grade K – 12. Vision testing will be done for all students grades K, 2, 4, 6, 8, and 10. Hearing testing will be done on grades K - 4, 6, 8 and 10.
These yearly screenings are done per New Jersey state law. The parent has the right to decline to have them performed in the school nurses office. If you choose to decline these screenings, please send in a letter for each child to the health office
